WAIVER: I know that lacrosse is a strenuous sport played with a hard ball and sticks, and presents certain inherent risks and hazards that I assume. I accept that All-Met, the directors, and staff of the camp assume no liability for injury or damages arising from the participation unless due to their willful fault or gross negligence. I know of no physical or other conditions, which would interfere with my child’s participation, and consent to emergency treatment for my child on my behalf.
